The battle of recriminations has begun. Who did what, when, in what particular order, who obeyed whom, or ignored instructions, was impatient, proved too lackadaisical? There will be much too much discussion, a thorough debate, arguments, resolutions, never any solutions. Once more the fog of war has descended and no amount of planning, codified procedures, forethought, would have altered on tiny circumstance.
The only way to resolve unnecessary mortality is to remove any means that assists to propagates the carnage.
